Discovery Chronic Care Program

The Discovery Health Diabetes Care Programme is available to almost all plans on Discovery Medical Scheme. Once a patient is signed onto the programme with Dr Murphy registered as primary provider for diabetes care, the patient is entitled to the following annual benefits:

To truly decrease the complications of diabetes, other risk factor conditions need to be well treated. Thus, Dr Murphy will also be responsible for the management of hypertension, hyperlipidaemia, and obesity. Dr Murphy, as a specialist physician, can look after all chronic conditions such as sleep apnoea, thyroid disease, mental health, and bone health. She will do this during the consultations allocated on the chronic care programme.
Dr Murphy and Sr Christine are available to their patients with diabetes for any queries telephonically or electronically. They particularly encourage patients who are trying to find correct insulin doses to send home glucose readings to them regularly. This support helps patients to get better diabetes control more efficiently.
