Managing diabetes is complex and challenging. At SWPMC we aim to help people living with diabetes to do this more efficiently and effectively. This will not only decrease the likelihood of diabetic complications but also improve overall quality of life. We give specific guidance on targets for glucose, cholesterol, blood pressure and weight, as well as advice and treatment to get to achieve this. We use both home glucose readings and laboratory parameters to monitor overall diabetes control. This structured approach should decrease anxiety by allowing the patient with diabetes to share their management burden with the diabetes team.

This is the program suggested for all patients with diabetes with some patients doing well with fewer visits and others requiring more frequent visits. Dr Murphy or Sr Manga can arrange referral to all the above.
